If you’ve ever found yourself standing at the top of Mount Bonnell, looking out over the Colorado River, or pacing yourself along the Ann and Roy Butler Hike-and-Bike Trail, you know that movement is the lifeblood of the Austin lifestyle.

But for many of our neighbors in West Lake Hills and the greater Austin area, that movement is shadowed by a voice in the back of their heads. Maybe it’s a voice that says, “Don’t push it, your back is fragile,” or “Your doctor said hiking isn’t a good idea with those knees.”

If you’ve ever felt deep down that your body is capable of more, but you’re held back by a “label” or a “warning,” you might be experiencing the Nocebo Effect.

What is the Nocebo Effect?

Most people know about the Placebo Effect—where a positive belief leads to a positive physical outcome. The Nocebo Effect is the opposite. It’s when a negative expectation—like a scary-sounding diagnosis or being told you have the “spine of an 80-year-old”—actually creates or intensifies real, physical pain.

In my 21 years here in Austin, I’ve seen how these “mental kinks” act just like a physical kink in a garden hose. When you believe you are “broken,” your brain sends signals to your muscles to brace and guard. This restricted flow—what we call a kink in the River of Life—is often what causes the very “static” and nerve pain you’re trying to avoid.

Challenging the “Austin Diagnosis”

We see it all the time at our studio on Bee Caves Road. A student comes in, longing to get back to the Barton Creek Greenbelt, but they’ve been living under a diagnosis that tells them they shouldn’t bend, twist, or reach.

These limiting beliefs are often the primary reason the “hose” stays kinked. The fear of movement becomes a self-fulfilling prophecy. You stop moving, the tissues get tighter, and the “static” in your legs or back gets louder.

From “Fragile Patient” to “Active Austin Student”

At CORE Therapy & Pilates, our mission is to help you “unkink the narrative.” We don’t want you to be a “patient” who is constantly waiting to be fixed. We want you to be a student of your own movement.

When you shift your mindset from “I have a bad back” to “I am learning how to restore my flow,” the nervous system begins to calm down. The bracing stops. The “River of Life” starts to move again.

Let’s Unkink Your Story

Have you been living by a diagnosis that feels more like a prison sentence? Are you avoiding the Austin landmarks you love because you’ve been told your body can’t handle it?

Let’s find out if those limiting beliefs are the real source of your pain. We invite you to step into our West Lake Hills studio for a Movement Assessment. Let’s look past the scary labels and see how we can get you back to the trails, the water, and the life you love.

Stop living by a label. Start living by your potential.

In my 21 years of practice at CORE Therapy & Pilates, I’ve seen thousands of MRIs, X-rays, and clinical reports. But there is one factor that shows up in nearly every room that no imaging software can catch.

It’s called the Nocebo Effect, and if you’ve been struggling with “zingers,” chronic tightness, or pain that just won’t budge, it might be the biggest “kink in the hose” you’re facing.

The “Dark Twin” of the Placebo

We’re all familiar with the Placebo Effect—the phenomenon where believing a treatment will work actually makes it work. The Nocebo Effect is its frustrating twin. It’s when a negative expectation or a fear-based belief actually creates or intensifies physical pain.

Think back to the last time a well-meaning professional told you that your spine looks “degenerate,” your joints are “bone-on-bone,” or that you should “be careful” because you’re fragile.

While those words are meant to be informative, they often act as a neurological “warning shot.” Your brain hears that you are broken, so it goes into high-alert mode. It clamps down the muscles, restricts the “River of Life” (your blood flow and nerve signals), and creates a state of protective “bracing.”

Why Language is a Therapeutic Tool

At CORE, we are very intentional about how we speak. We believe that the moment you step out of the clinical room and onto the Pilates Reformer, you stop being a “patient” and start being a “student.”

Why the shift?

A Patient is someone who is passive, broken, and waiting to be “fixed” by an outside force.

A Student is someone who is active, curious, and learning the skill of movement.

When you view yourself as a student, the fear begins to dissolve. You aren’t “protecting” a broken spine; you are “exploring” your structural anchors. This shift in mindset tells your nervous system that movement is safe, allowing those “kinked” vessels to finally open up and flow.

Unkinking the Narrative

If you’ve been told you have a “bad back” or “weak knees,” those labels are likely contributing to your pain levels. The Nocebo Effect creates a mental “bracing” that is just as real as a physical muscle knot.

Our mission is to help you “unkink the narrative.” By combining expert physical therapy with the intentional movement of Pilates and Gyrotonic, we teach your brain that your body is robust, adaptable, and capable of deep healing.

Is Your “Diagnosis” Holding You Captive?

Have you been living under the shadow of a scary-sounding label like “Degenerative Disc Disease,” “Permanent Nerve Damage,” or “Chronic Pelvic Instability”? Do you find yourself avoiding the activities you love because you’ve been told—or you’ve come to believe—that your body is simply too fragile to handle them?

It’s time to find out if that diagnosis is the whole truth, or if the Nocebo Effect has been keeping you stuck in a cycle of bracing and fear.

At CORE Therapy & Pilates, we don’t just look at your scans; we look at your movement. We invite you to come in for a consultation to see if we can help you “unkink” that narrative. Let’s test the waters, explore your structural anchors, and see how much of that pain is a permanent fixture—and how much is just a “hose” waiting to be unkinked.

Stop living by a label. Start living by your potential.

You’re lying on the couch, the drugstore heating pad is cranked to “High,” and yet, that deep-seated ache in your hip or back just won’t budge. You feel a bit of surface warmth, but the moment you turn it off, the pain returns exactly as it was before.

If you feel like your current heating pad is just a “warm blanket” rather than a therapeutic tool, you aren’t imagining it. There is a scientific reason for your frustration: dry heat versus moist heat.

The Problem with “Dry Heat”

Most standard heating pads provide dry heat. While convenient, dry heat has a major limitation—it doesn’t penetrate deep into muscle tissue. Instead, it heats the surface of your skin, which often leads to a “baked” feeling rather than true relief. This is why you feel better while the pad is on, but notice no lasting change in your underlying tightness.

The “River of Life” Solution

At CORE Therapy, we talk about restoring the “River of Life” to your body. Think of your muscles and nerves like a garden hose: when they are chronically tight (especially the psoas and hip flexors), the hose gets “kinked.” This restricts blood flow and traps waste products, creating the sharp “static” or electric nerve pain many of our patients experience.

To unkink that hose, you need vasodilation—the expansion of blood vessels that brings fresh, nutrient-rich blood back to the area. Moist heat is superior here because it penetrates deeper, relaxing the “structural anchors” of your body and effectively soothing the nerves from the inside out.

Why We Recommend the Thermophore MaxHeat

We don’t endorse gadgets that sit in a closet; we only recommend tools that provide measurable results. We stand by the Thermophore MaxHeat Arthritis Pad because it solves the biggest problems with home heat therapy:

Automatic Moist Heat: It draws moisture from the air, providing the deep-penetrating benefits of moist heat without the mess of water or damp towels.

Intense, Therapeutic Temperatures: It reaches higher, safer temperatures than standard pads, offering the kind of deep-tissue relief usually reserved for clinical settings.

Weighted Coverage: Its 14″ x 27″ size is nearly twice that of standard pads, making it perfect for wrapping around your hips or covering the entire span of your spine.

If you’re ready to stop managing your pain and start restoring your mobility, the Thermophore MaxHeat is the perfect companion to your Pilates and therapy work.

Sharp shooting electric pain has a few explanations. We’ve all been there—you’re walking the Lady Bird Lake loop or simply reaching for a bag of groceries when it hits: a sudden, sharp, electric-like jolt that shoots down your leg or across your foot. It’s more than just an ache; it’s a high-voltage reminder that something in your body’s “electrical system” is under distress.

Think of your nervous system as a complex network of electrical wires. For these wires to function, they need a clear path and a constant supply of energy. Sharp, shooting, electric-like pain is often the result of a mechanical “kink” in the system.

The hip flexor complex—specifically the psoas and iliacus—acts as the structural bridge of your body. When these muscles become chronically tight or weak (a common side effect of our “sitting-heavy” culture), they can compress the nerves that travel from your spine down to your feet. This compression acts like stepping on a garden hose; it creates a backup of pressure that manifests as that sudden sharp shooting electric pain or electric-like strike.

When these “zingers” strike, the natural instinct is to assume you’ve “thrown out” your back. But as we recently discussed in a deep-dive session between Stephen Dunn (CORE Therapy & Pilates) and Alex Montoya (Pair of Specs), the source of that electric shock might be hiding in a place you’d least expect: your hip flexors.

<img fetchpriority="high" decoding="async" width="900" height="600" src="https://therapyandpilates.com/wp-content/uploads/sharp-shooting-electric-pain.jpg" alt="Sharp shooting electric pain.
The Anatomy of a “Zinger”

Sharp Shooting Electric Pain

The “Structural Kink”: Unlike a dull muscle ache, electric sensations are typically caused by mechanical pressure on a nerve. In many cases, a tight or weak psoas muscle acts as a physical obstruction, “pinching” the nerve path.

The Garden Hose Effect: Think of your nerves as hoses carrying the “River of Life” (blood and signals). When the hip flexor is tight, it’s like stepping on that hose; the pressure builds up until it releases as a sharp “zinger” or shock down the leg.

Misdiagnosis of the “Faucet”: Patients often seek treatment for their lower back (the faucet), assuming the electricity starts there. However, if the kink is actually in the hip or leg (the hose), spinal adjustments alone won’t stop the shocks.

Starving Nerves: Sharp pain is often an emergency signal from a nerve that is being deprived of oxygen and nutrients. When circulation is restricted by structural misalignment, the nerve “fires” sporadically, creating that electric-like strike.

Identifying the Source: Using FLIR Thermal Imaging, we can see exactly where the temperature drops in the limb. These “cold zones” often correlate directly to where the nerve is being compressed by the hip flexor or other structural anchors.

Why “Checking the Faucet” Isn’t Enough

Many people in the West Lake Hills area spend years chasing this pain by looking only at their lower back. They get MRIs of the “faucet” (the spine), but they ignore the “hose” (the nerves traveling through the hip and leg). If your hip flexors are the structural “kink” causing the pressure, no amount of spinal adjustment will provide long-term relief.

Restoring the “River of Life”

At CORE Therapy & Pilates, we believe in restoration over management. We don’t just want to dull the electric pain; we want to “unkink the hose.” Through a combination of targeted movement and FLIR Thermal Imaging, we can identify exactly where your “supply chain” is failing.

Whether it’s a structural issue we can resolve with specialized stretching or a circulatory issue identified by our thermal mapping, our goal is to restore the “River of Life”—the vital blood flow your nerves need to stop firing those emergency signals. Click here to book an evaluation.

Spring in Austin is a call to movement. Whether you’re training for the Cap10K, coaching youth soccer at Zilker Park, or finally hitting the Barton Creek Greenbelt after a long winter, your body is about to face a major shift in physical demand.

At CORE Therapy & Pilates, we want to make sure our West Lake Hills neighbors stay on the trails and off the sidelines. To help you prepare, Stephen Dunn (Holistic PT and Neuropathy Specialist) sat down with Alex Montoya of Pair of Specs for a deep dive into the mechanics of injury prevention.

The Mechanical Anchor: Your Hip Flexors

As Alex and Stephen discuss in the video, the most critical “bridge” in your body is the hip flexor complex (the Psoas and Iliacus). After a winter of sitting, these muscles often become tight, weak, or “dormant.”

Think of your hip flexors as a mechanical anchor. When they are dysfunctional, they don’t just cause “tightness”; they create a structural kink that pulls on your lower back and alters how your nerves communicate with your legs. Strengthening these muscles is the #1 way to prevent the “phantom” pains and compensations that lead to shin splints, knee strain, and lower back flare-ups.

Don’t Ignore the “Static”

As you increase your mileage or intensity this spring, listen to your body. Are you feeling a strange heaviness or “static” in your feet? Many Austin residents spend years treating their back for these symptoms, but the root is often a mechanical failure—what Stephen calls a “supply-chain” issue where blood flow isn’t reaching the nerves.

See the Proof Before You Play

We don’t believe in guessing with your health. As showcased in our conversation with Alex, we use FLIR Thermal Imaging to map your circulation and nerve health before you hit the trail. By identifying “cold zones” or structural imbalances now, we can use a synergistic approach—including Vibration and Light Therapy—to “unkink the hose” and ensure your “River of Life” is flowing freely.

When you experience burning, tingling, or numbness in your feet, the most common medical response is to look at the spine. While a “pinched nerve” in the lower back (like Sciatica) can certainly cause leg pain, many Austin residents spend years treating their back while their feet continue to wither. At CORE Therapy & Pilates, we use FLIR Thermal Imaging to solve this diagnostic puzzle and reveal the true source of your symptoms.

The Diagnostic Divide: Back vs. Circulation

The primary difference between a “back problem” and “neuropathy” often comes down to the Supply-Chain. Your nerves require a constant flow of oxygen-rich blood—what we call the “River of Life”—to function.

When it’s the Back: If a nerve is compressed in the lumbar spine, the “signal” is interrupted, but the local blood flow in the foot is often still healthy. On a thermal scan, the feet might still appear as healthy reds and oranges because the circulatory “hose” isn’t kinked at the extremity.

When it’s Neuropathy: This is a localized supply-chain failure. The blood vessels that feed the nerves in your feet have constricted or collapsed. This is where we see the dreaded “Black Toes” on our thermal camera—zones so cold that the tissue is effectively starving.

Why Misdiagnosis Happens

Misdiagnosis is common because the symptoms feel remarkably similar. However, treating the back when the problem is circulatory failure is like trying to fix a dry garden by repairing the faucet in the house while the garden hose outside is stepped on. If the blood isn’t reaching the toes, no amount of spinal adjustment will stop the nerves from “starving.”

The “River of Life” Solution

Our 90-day protocol is designed to address the specific environment of the foot. By utilizing Vibration Therapy to act as a mechanical pump and Light Therapy to recharge cellular energy, we reopen the pathways that have been shut down. We don’t just guess if it’s working; we watch the “Black Toes” turn back to vibrant red on the scan as the supply chain comes back online.

Stop Guessing—Start Healing

Understanding the “why” is the first step toward reclaiming your mobility, but you don’t have to navigate the science alone. If you are tired of “managing” the pain with temporary fixes and are ready to see if your circulation can be restored, let’s get your recovery started today. Instead of wondering if your nerves can heal or if your back is truly the culprit, let us show you the data. Contact our West Lake Hills clinic now to schedule your Nerve & Circulation Evaluation and take the first step toward turning the “River of Life” back on.

If you have ever found yourself staring at your feet, wondering why they feel like heavy ice blocks, or why a strange, electric burning keeps you awake at night, you aren’t alone. “Why are my feet numb?” is one of the most frequent questions we hear from our neighbors in West Lake Hills and Austin.

For many, the standard answer is to “manage” the symptoms with medication. But masking the pain doesn’t answer the question of why it is happening. At CORE Therapy & Pilates, we look at it differently: Numbness isn’t just a mystery; it’s a supply-chain failure.

The Map of Your Recovery: Thermal Imaging

Because you cannot see your nerves with the naked eye, it is difficult to understand why they are failing. We use thermal FLIR imaging to turn your invisible symptoms into a clear, objective map of your circulatory health.

Blood flow is the “River of Life” for your body. It carries the heat, oxygen, and nutrients that your nerves require to survive and repair. When that supply is cut off, the nerves begin to “starve” and send out false signals of pain, tingling, or total numbness.

What the Colors Reveal on a Scan:

Vibrant Reds and Oranges: This indicates healthy circulation where oxygen-rich blood is successfully feeding your nerves.

Deep Blues and Greens: These are “cold zones” indicating sluggish circulation. Your nerves are being deprived of the fuel they need to function.

The “Black Toes” Effect: In severe cases, the temperature is so low the camera cannot register a healthy range. This “black” signature represents starving nerves in a state of crisis.

What You Feel: One Foot vs. Two

The physical sensations you experience every day often match exactly what we see on a thermal scan:

Numbness in One Foot: This often feels like one foot is a “dead weight” or an ice block while the other feels normal. This contrast creates “static” in your balance, making simple walks on Austin trails feel unstable and dangerous.

Numbness in Both Feet: This usually manifests as a symmetrical “stocking” sensation or a deep, electric burn. When both feet show “Black Toes” on a scan, it is a sign that the body’s internal supply chain is effectively offline.

Restoring the “River of Life”

The goal of our 90-Day Nerve Healing Journey is to watch those black zones disappear. We don’t use manual therapy; instead, we use a synergistic at-home protocol designed to reopen the circulatory pathways.

By utilizing Vibration Therapy to act as a mechanical pump for blood flow and Light Therapy to “recharge” your cellular batteries, we can actually watch the heat return to your feet.

When the “River of Life” is restored, the toes turn from black to red on the scan. This is physical proof that your nerves are finally being “fed” again. Once the supply chain is back online, the “static” of numbness and pain begins to quiet, allowing you to reclaim your mobility and enjoy restful, burn-free nights.

Stop wondering why your feet are numb. See the proof for yourself.

If you live in the Austin area and have been struggling with burning, tingling, or a strange numbness in your feet, you’ve likely heard the word neuropathy. You might even know it involves nerve damage. But most people don’t realize that neuropathy is rarely just a “nerve” problem—it is a supply-chain problem.

Seeing the Invisible

Because you can’t see your nerves, it’s hard to understand why they are failing. That is where thermal imaging comes in. The images at the top of this post aren’t just art; they are a map of survival. In a healthy foot, blood flow carries warmth, oxygen, and nutrients to every toe. On a thermal camera, this looks like vibrant reds and oranges.

When you see “Black Toes” on a scan, it represents a “River of Life” that has run dry. The black and deep blue areas indicate temperatures so low that the tissue is essentially starving. When nerves are deprived of oxygen-rich blood, they begin to misfire, sending those “static” signals of pain and burning to your brain.

Restoring the Flow

At CORE Therapy & Pilates in West Lake Hills, we use these scans to show our neighbors exactly where the “kinks in the hose” are located. Our 90-day Nerve Healing Journey isn’t about masking these symptoms with pills; it’s about changing the environment of the foot.

By utilizing a synergistic blend of vibration therapy to pump blood, and light therapy to “recharge” the cellular batteries, we can actually watch the heat return to the feet. As the toes transition from black to red on the camera, it’s a sign that the supply chain is back online. When the nerves are finally “fed,” the pain finally begins to quiet down.

Want to Dive Deeper?

Understanding the “why” is the first step toward reclaiming your mobility, but you don’t have to navigate the science alone. If you are tired of “managing” the pain with temporary fixes and are ready to see if your circulation can be restored, let’s get your recovery started today.
You’ve done the hard work. You’ve logged the hours on the treatment table, pushed through the initial pain, and finally received the news you’ve been waiting for: “You’re ready for discharge.”

Typically, this graduation comes with a Home Exercise Program (HEP)—a personalized roadmap of movements designed to maintain your gains, prevent re-injury, and help you return to the activities you love. But for many, the transition from the supervised care of a physical therapist to the “living room floor” is where progress stalls.

Here is why your HEP is the most important part of your recovery, and why the environment where you perform it makes all the difference.

What Exactly is an HEP?

An HEP is more than just a list of stretches. It is a clinical strategy. After formal therapy, your body is in a “maintenance and growth” phase. The goal is to ensure the strength, mobility, and pain relief you gained in the clinic are sustained through:

Consistency: Keeping the neurological pathways active.

Progression: Slowly increasing load to build resilience.

Prevention: Addressing underlying weaknesses before they become new injuries.

The Problem with “Home” and “Commercial Gyms”

While it’s called a Home Exercise Program, the reality is that most homes aren’t equipped for clinical-grade recovery.

The “Living Room” Barrier: It’s hard to focus on pelvic stability or nerve health when the dog is barking or the laundry is calling.

The “Big-Box” Intimidation: Commercial gyms are often loud, crowded, and lack the specialized equipment needed for precise post-rehab work. Using a standard weight machine when you need the nuanced resistance of a Pilates Reformer can actually be counterproductive.

The Solution: A Clinical-Grade Sanctuary

This is why we created the Post-Rehabilitation Performance & Wellness Space. We believe that your recovery deserves a dedicated environment that bridges the gap between the doctor’s office and the fitness center.

1. Precision Tools for Precise Results

Many HEPs require specific resistance and support. Our space provides independent access to:

Pilates Reformers & Chairs: For controlled, low-impact strength.

The Trap Table & Ladder Barrel: For decompression and spinal health.

Medical-Grade Treadmills: For safe gait refinement.

2. The Right “Headspace”

When you walk into our Austin area facility, your brain switches into “performance mode.” This is a space curated by Physical Therapists, meaning you are surrounded by people who value precision over ego. It’s a sanctuary designed to help you transition from “healing” to “thriving.”

3. Protecting Your Investment

You’ve already invested time and money into your physical therapy. Think of our wellness space as your insurance policy. By performing your HEP on professional equipment, you ensure that every rep is safe and every movement is effective.

Take the Next Step

Don’t let your discharge papers sit on the kitchen counter. Give your body the professional environment it deserves to stay mobile, strong, and pain-free.

Lately, at CORE Therapy & Pilates in Westlake Hills, we’ve seen a recurring pattern among the clients visiting our clinic. Many come to us frustrated by “phantom toes”—that unsettling sensation where your feet feel like blocks of ice, or you experience persistent tingling that makes walking the Austin greenbelts feel heavy and unsteady.

The common assumption we hear is: “I think I just have poor circulation.” While blood flow is certainly a factor, what we are observing in our clinical assessments is that the story is often much more complex.

The Nerve Connection: More Than Just Blood Flow

While circulation is vital, your nerves are the primary messengers of sensation. In our clinic, we often find that what feels like a “lack of blood flow” is actually nerve compression or a lack of neural stimulation.

When nerves in your lower back, hips, or ankles are compressed or “trapped,” the signal to your toes becomes fuzzy. It feels cold, numb, or heavy because the communication line is frayed. This is a classic example of the fragmented care we see in the traditional medical model—treating the foot when the actual “short circuit” is happening in the spine or hip.

The CORE Approach to Restoring “Flow”

To address this, we’ve developed a two-phase approach that we use daily with our Austin clients:

Pinpointing the Compression: We use expert Orthopedic Manual Therapy to physically “un-trap” the neural pathways. By addressing the root cause—whether it’s a compressed nerve in the lower back or a tight muscle in the hip—we restore the physical pathway for both nerves and blood flow to reach your feet.

Activating the “Pump” with Clinical Pilates: Once the pathway is clear, we must keep it open. Clinical Pilates on the Reformer is our secret weapon. The rhythmic, spring-loaded movements act as a natural “pump” for your lower extremities, flushing the area with oxygenated blood and keeping those neural pathways stimulated and healthy.

Secure Your Gains and Keep Moving

The goal isn’t just to get the feeling back once; it’s to ensure your world doesn’t shrink because of a lack of mobility. That is why we emphasize our Wellness Program. Once we’ve restored that connection, having unlimited access to our state-of-the-art Reformers allows you to maintain that “pump” and secure your gains long-term.

If your toes feel like they are losing touch, don’t wait for the sensation to disappear entirely. Come see us in Westlake Hills for a comprehensive evaluation. Let’s determine if it’s circulation, nerves, or both—and get you back to the activities you love. Click here to schedule an appointment.
